American Pain Consortium’s Chronic Care Management program helps eligible patients manage chronic pain and work toward their overall health goals through additional care coordination and ongoing support.

The program is recognized and supported by Medicare and connects patients with dedicated Chronic Care Management Specialists who work directly with their providers to help coordinate care.

What Is Chronic Care Management?

Managing a chronic condition often involves more than individual appointments. Chronic Care Management provides an additional layer of support between visits by connecting patients with a dedicated healthcare team.

Through the program, patients can receive help coordinating their care, managing an individualized care plan, and addressing questions or concerns that may arise along the way.

Who Is Chronic Care Management For?

Chronic Care Management is available to eligible patients who have Medicare as their primary insurance and are managing two or more chronic conditions.

Each patient works with a dedicated specialist who serves as an advocate and coordinates with the patient’s healthcare team. Patients can build an ongoing relationship with their specialist as they work toward achieving their health goals.
