While referrals are not required, your specific insurance plan may require one to receive coverage for treatment at our clinics. The following information can help you prepare for your first appointment and consultation with us.

Make a list of your symptoms, especially those that seem unrelated to the reason for your appointment. Be as descriptive as possible about when they began and how they have impacted your life. Include any key personal information about yourself, including major life stresses, and recent life or lifestyle changes, and include your personal and family medical history. Make sure to include a list of all medications, vitamins, or other supplements you take, including doses. It may also help if you write a list of questions to ask your doctor.
