Didn't Receive The Text?
If you did not receive the text, you will need to complete the paperwork in person the day of your appointment. Please arrive 60 minutes prior to your appointment to allow adequate time to complete your paperwork.
Complete Paperwork Through The Patient Portal, & Complete Consents At Appointment.
Prefer to start the no worries. You may complete your paperwork via your patient portal, but will need to arrive no less than 30 minutes prior to your appointment to complete the necessary consents.
