New Patients

We’re so glad you’ve taken the first step toward finding relief from pain.
Our goal is to learn as much about you as possible so we can create a treatment plan tailored to your unique needs. Please review the steps below to complete your new patient paperwork.

Step 1: Provide Your Email and Cell Phone Number

When scheduling your first appointment, our scheduling specialist will ask for your email address and cell phone number.

This allows us to send you important appointment information and give you access to complete your new patient paperwork before your visit.


Step 2: Create Your Patient Portal Account

After your first appointment is scheduled, you should receive a link via email inviting you to create your Patient Portal account. For convenience, or if you cannot find the email, you may also select the Patient Portal link below and choose “Create Account” to create your Patient Portal account.

Important: When creating your account, please enter your information exactly as it was provided when your appointment was scheduled. This may include your:

  • First and last name
  • Date of birth
  • Email address
  • Cell phone number

Using the same information helps the Patient Portal connect properly to your patient account in our system. If different information is entered, your portal account may not link correctly, which could prevent you from accessing or completing your new patient paperwork online.


Step 3: Complete Your Paperwork Before Your Visit

We encourage all new patients to complete their paperwork before their appointment.

If you complete your paperwork in advance, please arrive at least 30 minutes before your scheduled appointment time to complete required consents and any remaining check-in steps.


Step 4: Text Message Reminders

If you have not signed up for the Patient Portal or completed your paperwork, you will receive a text message with a link to complete your paperwork and begin the check-in process.

These text reminders are sent:

  • 4 days before your appointment
  • 2 days before your appointment

Please use the link in the text message to complete your paperwork before your visit.


Step 5: Completing Paperwork In Person

If you do not complete your paperwork through the Patient Portal or text message link, you will need to arrive 60 minutes before your scheduled appointment time to allow enough time to complete your paperwork in person.
