Step 1: Provide Your Email and Cell Phone Number

When scheduling your first appointment, our scheduling specialist will ask for your email address and cell phone number.

This allows us to send you important appointment information and give you access to complete your new patient paperwork before your visit.

Step 2: Watch for Your Patient Portal Email

Shortly after your appointment is scheduled, you will receive an email inviting you to sign up for the Patient Portal.

Through the Patient Portal, you can complete your new patient paperwork online before your first appointment.

Step 3: Complete Your Paperwork Before Your Visit

We encourage all new patients to complete their paperwork before their appointment.

If you complete your paperwork in advance, please arrive at least 30 minutes before your scheduled appointment time to complete required consents and any remaining check-in steps.

Step 4: Text Message Reminders

If you have not signed up for the Patient Portal or completed your paperwork, you will receive a text message with a link to complete your paperwork and begin the check-in process.

These text reminders are sent:

• 4 days before your appointment
• 2 days before your appointment

Please use the link in the text message to complete your paperwork before your visit.

Step 5: Completing Paperwork In Person

If you do not complete your paperwork through the Patient Portal or text message link, you will need to arrive 30 minutes before your scheduled appointment time to allow enough time to complete your paperwork in person.
